SECONDARY TRANSITION INTERAGENCY PLANNING MODEL

Purpose: The CIRCLES model promotes interagency collaboration and service coordination to assist transition-aged youth with disabilities to successfully transition to life after high school. Specifically, CIRCLES meetings offer students, schools, transition-related services agencies and families a unique opportunity to discuss a student’s individualized interests/needs, and to identify available resources to meet those interests/needs in the areas of education, employment, and independent living.

Who it serves: DC transition-aged (14-21) youth with disabilities

Power With: District of Columbia Office of the State Superintendent of Education (OSSE), District of Columbia Public Schools (DCPS), District of Columbia Public Charter Schools, and the District of Columbia Secondary Transition Community of Practice
